Medical Records for Hospital Patients

Please complete ALL portions of this authorization. Please specify if you would like to pick up the copies in person or have them mailed. Unfortunately, we do not fax copies of patient medical records to non-healthcare clinicians.

Note, requests for medical records are processed in the order they are received. Please allow up to 10 business days to process your request.

To expedite the process, you may fax the completed form and a copy of the patient’s valid photo ID to the Saint Mary’s HIM Department at 775-770-3679. Alternatively, you can bring a copy of the patient’s valid photo ID to Saint Mary’s Hospital 235 W. Sixth Street in Reno, NV.

BIRTH CERTIFICATES

Please note that Saint Mary’s Regional Medical Center does not have access to print birth certificates/certificate of live birth. If you were born at Saint Mary’s Regional Medical Center and are looking to obtain a birth certificate you will need to contact:

BIRTH CONFIRMATIONS

If you recently had a baby and you are looking for your birth confirmation/proof of birth, please allow up to ten days after birth to receive it in the mail if you were not provided one while still in the hospital. If you still have not received it after ten days, please call 775-770-3248. Please note that birth confirmations will not be provided after six months and you will be required to obtain a birth certificate instead.
